Activity: Root Cause Analysis of Sustainability Issues
Objective
Issue prevention is concerned with correcting root cause issues as opposed to addressing the symptoms. Issue prevention addresses issues at the source, preventing quality issues from flowing to downstream systems. Issue prevention is a key to efficiency, cost reduction and re-work.
Root cause issues that are identified as part of this activity should be tracked using the formal method established by the Sustainability Governance team. This process will be revisited as part of the Continuous Improvement Activities conducted in Phase 5.

Prevent Issues related to Systems and Business Process and Supply Chain
Objective:
Poorly designed systems and processes can result in issues that impact on sustainability. Business Process problems can result in the same process being performed more than once, steps being done manually as opposed to automated, conflicting work efforts or needless waste. Identifying the root causes of these issues and re-engineering business processes can have a major impact on improving overall efficency. The most significant improvements can often be made without technology changes. Some sustainability issues stem from fundamental problems in the architecture or issues related to the complexity of the integrating information in a highly federated environment.

Prevent Issues related to Overall Business Objectives
Objective:
When evaluating the root causes of sustainability issues all ideas should be evaluated, even high level causes such as society's consumeristic nature. Try to relate this to your product and service and see where you could reduce this impact. Could the product be made to be reusable? With less packaging? Smaller size? Look to areas within your direct or indirect control that you could make an impact on purchasing and disposal patterns.
Some of the key root causes of sustainability issues are the burning of fossil fuels. While organizations may not be able to solve this problem they can influence by reducing demand through efficiency and increase use of renewable energy.

Summarize Root Cause Issues and Recommend Changes
Objective:
After Sustainability issues have been identified, it is time to define a program of work for their resolution. Issues should be summarized and prioritized, based on the impact they have on in-scope metrics. As addressing root cause changes may involve initiating another scope of work at this time and significant changes may need to be made to the plan.
